In 4 simple steps to your treatment request:

  • 1. submit treatment request (see below: REGISTRATION or LOGIN).
  • 2. create samedi patient account or log in (if already existing) →  confirm samedi email (please also check your SPAM folder).
  • 3. fill out treatment request form online in samedi patient account.
  • 4. securely send us documents and details about the request directly in the form.
